Working with Forms
Your day-to-day activities within DocuPhase may involve the completion of web forms. These forms are generally easy to navigate and complete. However, the following information is provided to answer any questions you may have about completing Forms.
NOTE
While you can see completed Forms in the Viewer, Viewer Tools and Annotations are unavailable for use with Forms.

Web Forms greatly increase efficiency since they can do any or all of the following:
- Be made available to internal and/or external recipients for completion- including a digital signature.
- Capture data that can be stored in a database, and/or trigger automated processes
- Regulate and standardize consistent, trackable processes, while ensuring data integrity, compliance, and security
- Connect to other systems (including other DocuPhase components, external software programs such as Dynamic GP, websites, portals, and email) and databases within (and across) your organization for multiple users.
- Eliminate of manual processes via pre-population of fields (using with existing data)
- Attach files/documents to forms that can then be stored in the Repository*
- Automatically validate entered data
*Attachments are generally not seen from within the Viewer, nor are they Indexed.

Access to Forms depends upon a combination of settings:
IMPORTANT!
The Application Access Permissions and Application Form Visibility settings are dependent upon each other in order for a user to see/have access to a form. Be sure to review the table above to find out what settings need to be made to allow a specific user to have the appropriate access (Read-only, edit, fill, etc.). It won't matter what you set as the Visibility option if the appropriate access has not been given at the Application level, and vice versa.
- Access to the Application in which the Form resides: does the user have access to this Application. If so...
- Permissions for Application Forms: does the user have permission to create/edit, fill in, and/or delete forms for the corresponding Application

Once Permissions have been set for a user, access to individual Forms is then based on the Visibility setting; as noted in the previous section, Permissions and Visibility work in concert in determining who can do what with a Form. Visibility is determined at the Form level, usually when a Form is created. However, Form Visibility can be changed at any time. Visibility options are as follows:

EXAMPLEIf a form is created for Employee Annual Reviews, and associated with the Human Resources Application, only Users/Groups with Permissions to access the Human Resources Application have access to the forms (based in combination with other settings as described above. |

give access to the form to anyone logged into your DocuPhase system. NOTEThis option is not dependent upon Application Access or Form Settings. EXAMPLEYou may have a Supply Requisition Form that everyone in your company needs to be able to access in order to request office supplies, etc. All they have to do to have access is to log into DocuPhase. |
Global |
make a Form available via URL: anyone with access to the URL has access to the Form. Such users cannot be initially identified. The URL for such a form can be made available in an Email or from a website, etc. NOTE
EXAMPLELet's say a Form is used as a kind of portal where users can access the form to check on an order or personal information (e.g., patient records, financial information, etc.). They can enter credentials that are saved; thus creating a kind of account. This allows them to come back in and "sign in" using those credentials. |

- Read-Only - Forms may be set up so that they appear in read-only format: basically, as an image rather than an interactive/fillable form.
- Manual Entry Some forms allow you to enter/select information in easily identifiable fields.
- Auto Populate (Index Form Fill) - These forms may be created so that some (or all) of its form fields pre-populated with Index values from a selected document residing within the DocuPhase Repository. These are usually forms that are used over and over again, using the same information: thus preventing you from having to re-enter the same information time and time again (e.g., monthly expense reports might include your name, department, and supervisors name; purchase orders might include a vendor, vendor address, payment terms, etc.).

Specialized training is required to create, edit, and delete web forms in DocuPhase. Therefore, the creation and subsequent design of a Web Form within DocuPhase are generally done by a DocuPhase TPOC or System Administrator.

You can use the All Forms screen to find any fillable Form you want to complete/fill-in (e.g., Time Off Request, Purchase Request, etc.). Unlike completed Forms that you can Search for (and that appear in the Viewer), fillable Forms appear directly under the main header.Tell me more...

- By Application
- By Category
- As Favorites
IMPORTANT!
Not all Forms belong to a Category. Therefore, if you don't see the Form you need listed in any Category, try looking for the Form by Application, as described below.
TIPS
- Viewing by Category can filter the results to make it easier to find the Form you need.
- Regardless of the way you view Forms (i.e., by Application, Category, or Favorites), when you find the Form you want, click on the name of the Form to display it under the main header .

EXAMPLELet's say there's a dropdown list of over 300 vendors. It would be time consuming to scroll through a list of those 300 idenfiers. However, a ComboBox allows you to enter the first few characters of a vendors identifier (name, ID number, etc.). view specific forms that have been generated within the DocuPhase Platform. This is generally used when you are being asked to view a form for accuracy.
EXAMPLELet's say Security Training is required at your company. The trainer for that course may complete a form that is converted to a Certificate of Completion for each of the attendees that completed the course. The Forms Viewer is used to let each attendee review their own certificate to verify accuracy (such as date of course, spelling of name, association with the correct department, etc.). |
